HOW TO GET AN APPOINTMENT:

We accept most all insurances. Some insurance companies require physical or occupational therapy services to be referred or “ordered” by one of the following; your medical doctor, chiropractic doctor, podiatrist, naturopath, osteopath, or dentist.

Your doctor will write you a prescription for physical therapy or you can copy our prescription form and have them sign it. Some insurances will allow you to come to our office and they will pay for services without the referral. 

It is for insurance purposes only, that you need a doctor’s referral. If you are paying our fees without insurance benefits, you do not need a doctor’s referral to be seen.
We are located at 9430 Vancouver Mall Drive, Vancouver, WA 98662. We are in the Evergreen Plaza complex.

We are members of the American Physical Therapy Association and the American Occupational Therapy Association.
